Area: Wrenwood (Hotel 1st Floor)

Wrenwood City Guide: At the start of the game, go straight ahead and examine the city map located along the sidewalk.
Investigation Report: This file is available in the inventory when starting the game. Open the folder to read its contents.
Hotel Pamphlet: Documents lying on the floor in the middle of the hotel lobby.
Notice of Closure: The file is in the desk drawer inside the Office.
Alyssa’s Journal: This item is in an envelope hidden behind the painting in the Lobby. The file only appears after activating the cutscene in the bedroom.

Area: Wrenwood (Hotel 2nd Floor)

Report on Victor Gideon: The file will automatically be added to your inventory when you start Leon’s opening event sequence.

Rhodes Hill (Care Center)

Area: Care Center (3rd Floor)

Satiety Suppression Medical Records: The note is on a table in the attic.
Basement Safe Code: Located in the same warped door in the attic, near the Pressure Compensator accessory.

Area: Care Center (2nd Floor)

Progress Notes: Found on the table in Room 203 when starting with Grace at the Care Center.
Note on Cardboard Box: When entering Room 202, open the cardboard box in the closet to get this note.
Folded Notes: The file is in a doctor’s coat at the Gallery.
Chairman’s Note: Placed on the table in the middle of the room in the Chairman’s Office.
Copy of an Email to a Manager: Located on the table near the Moon puzzle in the Chairman’s Office.
Note to a Chairman: Found in the trash behind the Chairman’s desk.
Lead Researcher’s Message: Located on a cabinet in the Lead Researcher’s Office at the East Wing save point.
Lead Researcher’s Picture 1 / 2 / 3: The three photos are all on the desk in the Lead Researcher’s Office.
Examination Room Safe Code: The note is on a bloody locker in the hallway next to the Lead Researcher office.
Patient Medical Records: The file is on the table in the meeting room.
Spencer’s Research I: Received in the leather notebook after solving the Sun Puzzle in the Lead Researcher’s office.
Unlocking the Organ Transport Box: Located in the leather notebook after solving the Sun Puzzle.
Background Check: This file is on the desk in the office on the second floor of the West Wing.
Disposal Team Report: Found on cartons in the Storage Room.
Dishwasher’s Note: The note is behind the warped door at the end of the hallway outside the Chairman’s Office. The content reveals that the Food Storage Key is at Chef’s place if he has not been destroyed.

Area: Care Center (1st Floor)

Care Center Pamphlet: Found on the desk in the Security Office, opposite the typewriter.
Whiteboard Scrawl: Obtained by interacting with the whiteboard near the Pharmacy entrance.
Blood Compound Synthesis: Located on the table in the middle of the Blood Lab.
Safe Replacements: Located next to the opened safe in the Waiting Room.
Victor’s Journal Entry: Found in the locked security room at Blood Lab. You need the Identification Wristband (Level 1) from the corpse outside Lead Researcher’s Office (Level 2).
Construction Notice: Located in the hallway outside the Manager’s Office in the West Wing, on the way to the Garage.
Double Mutation: This note is located on crates in the Quarantine Area.
Nurse’s Log: Found on a table in the quarantine area.

Area: Care Center (Basement)

Staff Member’s Last Words: After jumping into the basement, walk a few steps forward to find this note next to a corpse.
Operation to Eliminate “The Girl”: Located in the first Security Room – Save Point of the basement.
Unlocking the Cells: Instructions are located on the power distributor in the cell block.
Technician’s Note: Located on the table next to the statue of Mr. Raccoon in Workshop.
Security Guard’s Report: Found near the safe in the basement, after activating the Power Distributor in Furnace.

Area: Care Center Courtyard (Surface)

Visitor Record: Found on the table near the door next to the helicopter.
Note Regarding Research Results: Located on the table in the VIP room.

Area: Care Center Courtyard (Facility)

Spencer’s Research II: The file is on the table next to the Item Box in the Reception area.
Subject 170 Record: Records are on a stretcher north of the Sample Collection & Examination Room.
Grace Ashcroft Blood Analysis Report: Found on top of the table in Private Lab.

Raccoon City

Area: East Raccoon City (Ground)

Report on the Raccoon City Incident: Automatically received when starting Leon in East Raccoon City.
Raccoon City Incident Newspaper: Newspaper on the ground near the starting position.
Orders for the Engineer Corps: Located in the BSAA tent at Central Camp.
Inventory List: Located in the BSAA container after leaving Cedarbrook Apartments via the underground parking lot.

Area: Raccoon City East (Lower Level)

Battery Storage Locations: This note is posted in the fuse box in the flooded area of the underground parking lot.

Area: Raccoon City East (Roof)

Engineer Corps Note: Located on the roof of Cedarbrook Apartments next to the golden crate containing the Broken Signal Receiver.

Area: Raccoon City Center (Ground Floor)

RPD Perimeter Report: Located on a wooden crate in the Main Hall opposite the laptop, revealing the location of W870 Police Shotgun and Silver Scope.
Top Secret Operations: Located on Lieutenant Marvin Branagh’s desk in the West Office.
Record of Events: Paste on blackboard in Operations Room.

Guide Pamphlet: After escaping from Mr. X in the Press Room, found this file on a bench on the second floor of Main Hall.
Library Card: Located in a drawer at STARS Office. Open the drawer and wait a few seconds for the card to fall out.
Barry’s To Do List: Located on the table in the middle of STARS Office.
Scavenger Hunt – Clue 1: Pictured in his Made In Heaven jacket after shooting down the ladder in the Reception Room.
Scavenger Hunt – Clue 2: The second photo is in a nearby cabinet.

Toe Tag: Located in a coffin in Catacombs.

Series 60 Results: Located on the table in the Research Room after exiting the Underground Chapel.
New Researcher’s Notebook: Located on the table in the Research Lab after escaping the Underground Chapel.

Area: ARK Facility (Top Floor)

N0-AH Status Report: Located in the Staff Room on the top floor of the ARK Facility, after exiting the Decontamination Tunnel.
Product Catalog 1 (2026), Raccoon City Bioweapon Research – Report, Product Catalog 2 (2026): The three files are all on the Living Room wall, after the cutscene in the Server Room and the Warehouse battle.

Access Log: 51st Assembly Minutes: Located in the Monitor Control Room near the Lobby.
Access Log: First Assembly Minutes: After unlocking the Archive, interact with the rightmost screen before using the Main Computer.
Alyssa’s Interview Notes: Automatically received when playing with Grace again in Archive.
Letter from Patrick: Automatically received when playing with Grace again in Archive.

Area: ARK Facility (Basement 3)

Sterilization Chamber Safe: Located in a wooden crate under the Waiting Room in front of Bioweapon Storage 05.
Monitor Control Room Safe: Located in a wooden crate at Bioweapon Storage 05.
